According to investigators, the Cessna pilot acknowledged a request to extend his downwind but later turned toward the runway while the helicopter was still operating in the runway environment. Surveillance footage shows the Cessna never actually touching down, instead making several unusual movements just above the runway before pitching up and ultimately colliding with the helicopter. The NTSB also says investigators have not found evidence of a pre-impact mechanical failure that would have prevented either aircraft from operating normally.
