On September 5th 2021 the Haunted Mine Drop at Glenwood Caverns Adventure Park was operating normally. At around 7:10pm a 6 year old rider was loaded along with 5 other riders into the drop tower. The ride operators checked the restraints of all riders, including the involved rider who was sitting on top of their seatbelt with the tails of the seatbelt over their lap.
