No, we don't start with 1 HP, we can only at most have 1 HP. How? I modded an item into the game that ends up removing 150k HP - just to be sure I won't ever get it back to positive values. The insanity behind the challenge is that all those moments you dont think about getting hit as it doesn't really matter will now kill you. Walking through enemies to get material? Dead. Walking to a tree? Dead. Walking to a crate guy? Dead. In other words, you have to re-wire how you play the game if you usually aim for optimal performance.
