This was the pilots’ first time flying into this airport, and they were unfamiliar with the area. As the aircraft descended into the clouds above the island, a series of misunderstandings began to take place between the pilots and air traffic control. Minute by minute, as the aircraft descended, these misunderstandings were compounded, and the shared mental pictures between the pilots and air traffic control began to diverge, with disastrous consequences. Flight 1308 sent shockwaves across the aviation industry, and raised serious questions about everything from the way pilots and air traffic controllers communicate, to the way that approach charts are designed.
