Can somebody who trains hard five times a week still be quietly heading towards disease? In this video I put diet and exercise side by side and look at what each one can do on its own, from a major Mediterranean diet trial to the pooled evidence on physical activity and mortality, and then at the thing sitting underneath almost all of it, which is insulin resistance and how silently it builds. Both camps in this argument have genuinely strong evidence behind them, which is exactly why it has never been settled. Changing nothing but the food on your plate has been shown in a randomised trial to cut heart attacks and strokes substantially, and moving more has been linked to a large drop in the risk of dying across hundreds of thousands of adults. So the honest starting position is that both work, and anybody telling you one of them is a waste of time is not reading the evidence properly. Where it gets more interesting is what I have watched clinically over the last decade, which is that being fit and being metabolically healthy are not the same thing, and the two come apart far more often than most people expect. I explain why that pattern points towards food carrying the heavier hand, why the original question was framed badly in the first place, and what I would actually do about it if you are starting from scratch, including which blood tests are worth asking for and when to recheck them.
